ED Management
ATLS protocols should be followed to diagnose and treat other life threatening injuries, which are common. Hemorrhagic shock is common and is a major priority. To reduce hemorrhage into the pelvis, the unstable pelvic ring can be stabilized with a pelvic binder (also known as a T-POD), bed sheet wrap, or an external fixator. When available, interventional radiology can angiographically embolize many pelvic vessel hemorrhages. This would require an early call to the interventional radiologist to prepare the angiography suite, as time is often critical. Orthopedics should be involved early for all unstable fractures.
